Springfield~ Rafael Ortiz Pantoja, 38, passed away on February 15, 2015. Rafael was born in Springfield, MA on December 15, 1976 to Rafael Ortiz and Carmen Pantoja. He leaves to cherish his memory: his son, Christopher J. Pantoja; his daughter, Alayjah Pantoja; his parents; and his 3 sisters, Catherine Ortiz, Mercedes Ortiz and Carmen Pantoja. He will be greatly missed. The Funeral Services will be held on Monday, March 2, from 9 - 11 a.m. at Puerta Del Cielo® Funeral Home, 730 State St., Springfield, followed by Burial in Oak Grove Cemetery.
